Speech & Language Pathologist - Home Health - (SLP - HH)

Pay Rate: $2083.02 - $2282.46
Hello, Position Title: SPEECH PATHOLOGIST Total number of positions needed: 1 All of the info below must be completed before I can open the req. 1. Who will be the DOO approving the contractors time each week? 2. What is the iCIMS req # corresponding to this requisition? 3. How long is the contract assignment? 8-9 weeks is the norm: 4. Schedule of assignment hours? M – F 8am – 5pm? Monday-Friday 8am-5pm 5. Will there be any weekend rotation? If so, how often? no 6. Will there be any on call? If so, how much? no 7. What region are you in? Home health or Hospice? NE, home health 8. What territory this contractor will be asked to cover? The entire coverage area for Midlothian Care Center-this will include Petersburg areas, Richmond areas, Powhatan areas, Glen Allen areas, Mechanicsville areas 9. What’s creating the request? - work load Ect.. current ST is out on FMLA for 10 weeks 10. 11. Which location will this person report to for their assignment- add info below Full address: 4589 Lifestyle Lane Midlothian, VA 23112 OU# (location code) 1704/J99 12. About how many miles per week will the contractor travel? 200-250 miles 13. What are the weekly units of productivity expectations? Ex: 25/30 32 unites 14. What EMR systems will the consultant be using: Will only use point of care.hchb 15. Do you want the candidate to have experience in have home health, hospice or both ? home health
