Job Responsibilities: Evaluate and assess speech, language, cognitive-communication, and swallowing disorders in residents of skilled nursing facilities (SNF). Develop and implement individualized treatment plans to address communication and swallowing deficits, enhancing residents' quality of life. Provide direct therapy services, including individual and group sessions, to improve speech, language, and swallowing functions. Educate residents, families, and caregivers on strategies and techniques to support communication and swallowing needs at home and in the facility. Collaborate with interdisciplinary teams, including physicians, nurses, occupational therapists, and physical therapists, to ensure comprehensive care for residents. Conduct regular progress assessments, adjusting treatment plans as necessary to meet residents' evolving needs. Document evaluations, treatment plans, progress notes, and discharge summaries in compliance with healthcare regulations and facility policies. Participate in care planning meetings, providing expert input on residents' communication and swallowing needs. Stay current with advancements in speech-language pathology, participating in continuing education and professional development opportunities. Adhere to ethical and professional standards, ensuring compliance with all applicable state and federal regulations. Job Requirements: Master’s degree in Speech-Language Pathology from an accredited program. Current state licensure or certification as a Speech-Language Pathologist (SLP). Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) is preferred. A minimum of 1-2 years of experience in a skilled nursing facility or geriatric setting. Strong knowledge of speech, language, and swallowing disorders, particularly in the elderly population. Excellent communication and interpersonal skills. Ability to work effectively as part of an interdisciplinary team. Strong organizational skills and attention to detail. Compassionate and patient-focused approach to care.
