Learning Outcomes
- After this course, participants will be able to describe the SLP’s role in sustainable discharge planning within the client’s interprofessional team, including considerations and strategies for environmental modification and/or safety in the home.
- After this course, participants will be able to describe how to evaluate person-centered environmental factors within the scope of practice of the SLP related to hearing impairment, swallowing, and cognitive-communication that promote quality of life, independence, and aging in place.
- After this course, participants will be able to describe how to apply concepts of interprofessional collaborative practice to a service delivery model integrating SLPs into home safety and modification utilizing a case study approach.

Stacie Delezene, MS, CCC-SLP is a speech-language pathologist and Clinical Director with Powerback. Stacie has worked in a variety of post-acute settings for over 10 years, and has received a Master Clinician recognition with Powerback in Technology, and Cognitive-Communication Disorders. She has presented at the university, state, and national levels on topics relating to dementia care, mentoring in post-acute care, chronic disease management, and interprofessional practice. Stacie has a passion for patient advocacy, and also enjoys learning new ways to integrate technology into evidence-based interventions, in order to promote sustainable outcomes and quality of life along the continuum of care.
