It was well organized and provided a framework for breaking down very complex medical cases. It was helpful to establish specific milestones and common risk factors at each milestone of the weening process when decisions about speaking valves and PO intake might be made. The case study was very helpful for putting that theoretical decision-making system into practice. The instructor was knowledgable and the handouts/slides were helpful to look back on later.

What made it excellent? Two things. One, info that actually impacts therapy. Info that was new to me! Specifically, the pip/peep stuff was new! Good info. Two, the "frameworks" for thinking...i.e. making flowcharts, avoiding bias, adjusting...basically, metacognition about learning. If a therapist has that, mixed with care for the patient, the ship will right itself, mistakes will be minimized as much as possible, the therapist will improve. Good stuff. Not everyone thinks like this.

Decision making charts about patients with trachs seems good in theory, but I feel it would be difficult to implement in an acute care setting where there is a push to get patients moving to a different level of care or discharged quickly. My experience is that doctors push therapists to do PO trials quickly and I'm not sure they would accept such a rigid flow chart.
