I've had misconceptions about AP since grad school. I didn't realize the lungs had so many defense mechanisms and that a lot actually has to occur before a pt develops AP. I loved the analogy that we are "hammers". I do have a tendency of hearing someone cough and immediately worrying that it's due to aspiration. I know now to be more open minded, look at risk factors and the patient as a whole before assuming it's an issue I need to immediately address.

The speaker challenged me to think outside the box and to look at the person as a whole and not just their swallow/possible dysphagia. I also liked that he spoke on multiple risk factors for aspiration pneumonia, and ways we (as SLPs) can minimize the overall risk.

Some of the information seems biased towards the presenters personal practice style in management of complex cases. I don't necessarily agree. For example, I would not feel comfortable with continuing advancements towards PO in patients with multiple aspiration pneuomonia risk factors while 'referring out' to get respiratory treatments (albuterol or lung clearance etc.) Pneuomonia shouldn't be the only health outcome to focus on either; overall pulmonary health should be seen holistically too.

I really enjoy learning from Mr. Barnes, as I always learn something important and he makes me think more about the patient as a whole and not just as an evaluation/treatment. This was an excellent course to encourage the confidence of the SLP in LTC/SNF with understandable material. I especially felt validated with encouragement that SLPs can/should be reserved at times as we do get significant input (i.e., pushback) with lowering diet levels. In addition it was lovely to here we ARE important and should be interacting with many other interdisciplinary team members not just our basic PT/OT staff.
